These Terms govern text message communications sent by or on behalf of Francisco Kim DDS Inc., operating as Kairos Dental Group (Anaheim and Los Angeles locations), to patients and prospective patients who have provided consent to receive such messages.
Kairos Dental Group sends text messages through Nextiva and Open Dental for the following purposes:

You may stop receiving text messages at any time by replying STOP to any message. After opting out, you will receive one final confirmation message and no further texts. To re-enroll, text START or contact our office directly.

Standard message and data rates may apply depending on your mobile carrier plan. Kairos Dental Group does not charge separately for text messages.
Carriers are not liable for delayed or undelivered messages. Message delivery is subject to effective transmission from your mobile carrier. Kairos Dental Group and its messaging providers (Nextiva, Open Dental) are not responsible for messages that fail to deliver due to carrier-side issues.
Most major U.S. carriers are supported, including AT&T, Verizon, T-Mobile, Sprint, Boost, and others. Not all features are available on all carriers.
Mobile phone numbers collected for SMS purposes are not shared with third parties for marketing purposes. See our Privacy Policy and HIPAA Notice below for full details on how we handle your information.
You are not required to consent to receive text messages in order to receive dental care from Kairos Dental Group. Consent is entirely voluntary.

This Privacy Policy describes how Francisco Kim DDS Inc., operating as Kairos Dental Group ("we," "our," or "us"), collects, uses, and protects your personal information when you interact with our practice, website (kairosdentalgroup.com), or communications.
We may collect the following categories of personal information:
Mobile phone numbers collected for text messaging are used solely to send the types of messages described in Section 1 above. We do not sell, rent, or share mobile phone numbers with third parties for their own marketing purposes. No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
We do not sell your personal information. We may share your information with:
Patient records are retained for a minimum of 10 years from the date of last treatment (or age 19 for minors), in compliance with California law. Communications and contact information are retained as part of your patient record.
California residents may have rights under the California Consumer Privacy Act (CCPA), including the right to know what personal information we collect and the right to request deletion of non-health personal information. To exercise these rights, contact us at the address below. Note: health information is governed by HIPAA, not CCPA.
We implement administrative, physical, and technical safeguards to protect your personal information, including encrypted communications, access controls, and staff training on privacy practices.
We may update this Privacy Policy periodically. The effective date at the top of this page reflects the date of the most recent revision. Continued use of our services following a material change constitutes acceptance of the updated policy.

Francisco Kim DDS Inc., operating as Kairos Dental Group, is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). We are required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with this Notice.
This Notice applies to both of our locations:
Treatment. We may use and disclose your PHI to provide, coordinate, or manage your dental care. For example, we may share your records with a specialist to whom we refer you, or with a dental laboratory fabricating a restoration for you.
Payment. We may use and disclose your PHI to obtain payment for services rendered. This includes submitting claims to your dental insurance and verifying your coverage prior to treatment.
Healthcare Operations. We may use your PHI for internal operations such as quality review, staff training, scheduling, and business management.
Appointment Reminders. We may contact you by telephone, text message, or mail to remind you of scheduled appointments or to follow up after treatment. You may request that we contact you in a different way or at a different number.
As Required by Law. We will disclose PHI when required by federal, state, or local law, including for public health purposes, to report abuse or neglect, for law enforcement purposes, or in response to a valid court order.
Other uses and disclosures of your PHI not described above will be made only with your written authorization. You may revoke an authorization in writing at any time. We will not use or disclose your PHI for marketing purposes without your written authorization.
Right to Access. You have the right to inspect and receive a copy of your PHI in a designated record set. We may charge a reasonable fee for copies. Requests must be submitted in writing.
Right to Request Amendment. If you believe PHI in your record is incorrect or incomplete, you may request an amendment. We may deny the request under certain circumstances.
Right to an Accounting of Disclosures. You may request a list of disclosures of your PHI we have made, other than for treatment, payment, healthcare operations, or disclosures you authorized.
Right to Request Restrictions. You may request that we restrict use or disclosure of your PHI. We are not required to agree, except that we must agree to a restriction on disclosures to your health plan for services you paid for out-of-pocket in full.
Right to Request Confidential Communications. You may request that we communicate with you about your PHI in a specific way or at a specific location (e.g., a different phone number or mailing address).
Right to a Paper Copy of This Notice. You may request a paper copy of this Notice at any time, even if you previously received it electronically.
We are required by law to maintain the privacy of your PHI, to provide you with this Notice of our legal duties and privacy practices, and to notify you following a breach of unsecured PHI. We are required to abide by the terms of this Notice while it is in effect. We reserve the right to change the terms of this Notice and to make any new provisions effective for all PHI we maintain. A copy of any revised Notice will be posted in our offices and on our website.
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services Office for Civil Rights. Filing a complaint will not affect the care you receive from us.
